Feedback Form Problem

Ok... I need to create a feedback form?

Basically I have tried numerous feedback forms, none of which have worked. Has anyone got ANY idea how i can create a form, which then sends the information to my email address?

I would really appreciate it

Grrrr.... So annoying.

I need to set up a form, so someone can apply on my website. I would like the form to be submitted to an email address. It needs to include the following fields:

Name
Email
Application
Sample of work
Desired username
password
confirm password

Now ive hunted for Form Mail scripts, but so many of them are impossible to configure unless your really good at CGI or PHP.
Im good at Neither.

Can anyone help me out?
